Terms and conditions governing this quotation are attached hereto. Insurance requirements, if applicable, are also attached hereto as part of this document. As this price request constitutes an inquiry, and not an order, it implies no obligation to purchase on the part of Lake County.

Leather Fire Helmets

All prices submitted are to be on the form below in accordance with all terms and conditions set forth in this Request for Quotation. Prices quoted should be in unit of measure shown. Any award resulting from this RFQ will be made to the responsive, responsible vendor which offers the lowest price on an item basis. If award is noted to be made on an aggregate basis, any vendor response that fails to include pricing for all items may be rejected. Per Section 287.05701, Florida Statutes, the County will not request documentation of or consider a vendor's social, political, or ideological interests when determining if the vendor is a responsible vendor.

Prices shall be quoted F.O.B. Destination – inside delivery, freight included and shall be inclusive of all costs. Current and/or anticipated applicable fuel costs should be considered and included in the price quoted.

Delivery of items is to be within 90 days after any purchase order is issued.

Specifications and/or Special Conditions

Pricing shall be inclusive of all labor, materials, tools, equipment, fuel, freight, delivery, and other incidentals costs. Pricing shall include costs associated with vendor sizing assistance. Unit prices shall govern all services requested.

Anticipated number of helmets ordered annually is 8 to 12. Exact number is to be determined. 

If applicable, please upload a document if any volume-based price breaks apply and/or if proposing an equivalent model, please submit the brand, model, and specifications. A sample for review and evaluation is required for any proposed equivalent. The County makes the final determination if the equivalent qualifies. 

Any questions on this solicitation must be sent in writing to be answered in this section of the RFQ. Any questions must be received by close of business Monday, November 17, 2025.

Changes made as of 11/05/2025:

1. Modification to the specifications and requirements of RFQ, and Exhibit A. 

- Color, rank, and bend are to be determined at time of order.

- Colors: Battalion Chief- White, Captain- Red, Lieutenant- Red, Firefighter- Black and Flat Black

Vendor may upload one document with additional information. If applicable, please include any volume-based price breaks in the document. If proposing an equivalent model, please submit the brand, model, and specifications in the document. A sample for review and evaluation is required for any proposed equivalent. The County makes the final determination if the equivalent qualifies. 

QUESTIONS AND ANSWERS SUBMITTED ON 11/05/2025:

Q1. The different line items for the helmets all had different quantities but didn't specify what color coordinated with what line item, nor did it specify which bend you wanted on each line. 

A1. Additional line items have been removed. Please refer to Exhibit A, Section 1.2.

Q2. "Leather ID Shields: is listed on your bid, but nowehere on the bid did it request any shields, let alone quantity for each different rank of shields. 

A2. Please refer to Exhibit A, Section 2.1.14 and Leather ID Shields document.

Q3. I would also then need to know what color helmet the Captain would be getting, what color helmet for the Firefighter, the Lieutenant, and so on...

A3. Please refer to Exhibit A, Section 2.1.10.

Q4. Are you looking for a generic quote for one helmet and one front then and then applying that pricing to the 8-12 that is possibly going to be ordered?

If "yes" is specified above, the specific requirements are described within this RFQ. The vendor selected for award must provide a Certificate of Insurance that clearly complies with the stated insurance requirements prior to issuance of any purchase order. Failure to do so within the requested timeframe (five (5) working days under otherwise noted) may be cause for rejection of that vendor's response.
