Job Summary:
The essential function of the position within the Office of Housing and Community Services is to provide comprehensive and essential individualized intensive case management and supportive services to clients formerly experiencing homelessness. This role will provide program participants homeless crisis intervention, housing stabilization, working collaboratively with industry professionals to assist participants with additional supportive services which may include, professional and/or health care services, community outreach programs, and identified networks of support.
The Case Manager will be responsible for building rapport and maintaining positive and professional relationships with clients, landlords, service providers and other community partners. Maintaining accurately recorded documentation, files and data collection required by local, state and federal grants is a key responsibility as well as collaboratively developing a detailed individual housing stability plan with each participant.
The position will work closely with our Continuum of Care and under supervision of the Director of Housing and Community Services, developing work methods and sequences.
This position shall be required to assist with emergency support functions in the event of a declared emergency and/or any other emergency duties as assigned.

Minimum Qualifications:
Requires a Bachelor’s degree in a human services related field with three (3) years of progressively responsible related experience such as the Permanent Supportive Housing Program, the state (Florida), area services/community partners or federally funded grant programs and federal housing programs.
Requires a valid Florida Driver’s license and appropriate licensures or certifications in Homeless Management Information System (HMIS) within six months of hire.
