Spay/Neuter Rebate Submission Form

Instructions

To begin the application process:

  1. Download the rebate application form. Fill out the form and save it to your computer.
  2. Scan the following supporting documentation (save as PDF or as image: jpeg, tif, png, gif, etc.)
    1. A copy of receipt or invoice from a veterinarian stating the spay/neuter procedure was performed.
    2. Proof of full-time Lake County residency clearly showing the applicant’s name and physical address. Acceptable forms include a Florida issued ID, voter registration, or property tax bill with homestead exemption.
  3. Print out the application form and sign it (completed form must include your signature). Scan the signed form. (save as PDF or as image: jpeg, tif, png, gif, etc.)

To submit your application:

Use the form below to upload and send your application and supporting documentation to the Spay/Neuter Rebate Program. Make sure the following is included:

  1. Completed and signed application form
  2. Proof of Lake County residency
  3. Receipt/Invoice from a veterinarian that performed the sterilization.
